Working on a Disability Pension: How Much You Can Earn

This is a translation of the Japanese original. The Japanese version is authoritative; figures follow Japanese tax law.

A disability pension (shogai nenkin) does not stop just because you work and your pay goes up. The only one with an income-based suspension is the Disability Basic Pension for an illness or injury that first appeared before age 20. There are two lines, both drawn on last year's income: ¥3,761,000 and ¥4,794,000. Converted into salary, the half suspension begins a little above ¥5,250,000. What matters more than income is the medical certificate you file at renewal. Start by checking your pension certificate to see whether your pension is a before-20 case.

The short answer: only the before-20 Disability Basic Pension stops on income

"Work and your disability pension stops" does not apply to most recipients. In the Japan Pension Service's guidance, an income-based suspension appears on one page only: the one about illness or injury before age 20[Japan Pension Service]. The Disability Employees' Pension has no income test, and neither does a Disability Basic Pension whose first medical visit fell while you were enrolled in the National Pension.

Type of pensionIf you work and your income rises
Disability Employees' Pension (grades 1, 2 and 3)It does not stop
Disability Basic Pension (first visit while in the National Pension)It does not stop
Disability Basic Pension for illness before age 20Half or all of it is suspended, on last year's income

The clue that tells them apart is the first medical visit. If the day you first saw a doctor came before you turned 20, it is a before-20 case[Japan Pension Service, Disability Basic Pension].

The before-20 line is last year's income of ¥3,761,000

The before-20 Disability Basic Pension is paid for a disability that arose in a period when no contributions were made. In return it carries an income test, and a suspension runs from the October payment through to the following September[Japan Pension Service, payment limits for illness before age 20].

Above ¥4,794,000Fully suspended, ¥0Fully suspended, ¥0

Dependants push the line upward: ¥380,000 for each one, or ¥630,000 for a specified dependent relative.

The common mistake is to confuse "income" with "pay". Income is what is left after the employment income deduction is taken off your gross pay.

Does pay of ¥4,000,000 trigger a suspension? (this site's calculation)
  • Employment income deduction = ¥4,000,000 x 20% + ¥440,000 = ¥1,240,000[National Tax Agency No.1410]
  • Income = ¥4,000,000 - ¥1,240,000 = ¥2,760,000. That is under ¥3,761,000, so the pension is paid in full
  • In salary terms the line is crossed from about ¥5,250,000

Note: this site's calculation, applying the National Tax Agency's quick table.

With one dependant the line rises to ¥4,141,000. If a suspension notice arrives, check whether a dependant went unreported.

Working does not lower your grade automatically

If anything stops the pension, it is the renewal review, not your income. Even there, the Ministry of Health, Labour and Welfare's guidelines press the opposite point. They state that "the fact of being engaged in work shall not be taken, in itself, as showing that the ability to carry out daily life has improved"[MHLW, grade assessment guidelines].

  • Type A and Type B supported employment and disability-quota employment: "consider the possibility of grade 1 or 2"
  • Ordinary companies or self-employment with a comparable level of support: "consider the possibility of grade 2"
  • Ordinary companies: "judge the realities of the work as a whole"

There is also a brake on moving someone to a lower grade. The reviewer is required to look at the previous certificate and to query the attending doctor before deciding. Even so, the certificate is the evidence. Tell your doctor what adjustments your workplace makes for you.

Watch the renewal deadline too. The disability status report arrives at the end of the month three months before your birthday month, and the deadline is the last day of your birthday month[Japan Pension Service, disability status report]. File it late and payments pause. How often it arrives differs from person to person.

The timing differs by direction. A higher grade raises the amount from the following month; a lower grade cuts or suspends it four months later[Japan Pension Service]. Those four months give you time to rearrange your finances.

In fiscal 2026 the grade 2 Disability Basic Pension is ¥847,300 a year

For fiscal 2026 the basic pension rises by 1.9% and the earnings-related portion by 2.0%[Japan Pension Service, from the April 2026 payment]. Grade 2 equals the full Old-age Basic Pension, or ¥70,608 a month.

If your Employees' Pension coverage falls short of 300 months, the calculation treats it as 300 months[Japan Pension Service, disability pension guide]. A short record does not leave you with an extremely small amount.

Grade 2, average standard remuneration ¥300,000, treated as 300 months (this site's calculation)
  • Earnings-related amount = ¥300,000 x 5.481 / 1000 x 300 months = ¥493,290
  • ¥847,300 + ¥493,290 = ¥1,340,590 a year (¥111,716 a month)
  • With a spouse under 65, the addition of ¥243,800 brings it to ¥1,584,390
  • Grade 1 would be ¥1,675,738; grade 3 would be the minimum ¥635,500

Note: this site's calculation, applying figures to the formula in the "Disability Pension Guide, fiscal 2026 edition".

For grade 3, the minimum kicks in below an average standard remuneration of roughly ¥386,000.

The pension does not stop, but you do come off a family member's dependent status

A disability pension is tax-free and is not counted in your total income. The basis is not the Income Tax Act but the ban on public charges in Article 25 of the National Pension Act and Article 41(2) of the Employees' Pension Insurance Act[National Pension Act, Article 25]. Only old-age pensions are taxed.

The problem is your pay. For 2026 the income requirement for a dependent relative is total income of ¥620,000 or less[National Tax Agency No.1180]. Pay of ¥1,500,000 gives employment income of ¥760,000, which is over that line. The pension keeps coming, yet you come off the dependent status.

What happens on the pension side

  • Unless it is a before-20 case, the pension is paid in full whatever you earn
  • Grade 1 or 2 brings a statutory exemption from National Pension contributions

What happens on the family's tax side

  • Employment income above ¥620,000 ends your status as a dependent relative
  • The disability deduction is decided by your handbook, not by your pension grade

That last point is widely misunderstood. What the National Tax Agency lists as requirements is a mental disability certificate, a physical disability certificate or a certification by a mayor or equivalent. The grade of your disability pension does not appear[National Tax Agency No.1160]. The amounts and the requirements are set out in our guide to the single-parent and disability deductions.

The statutory exemption is worth using. A Category 1 insured person receiving a grade 1 or grade 2 disability pension is exempt from contributions[Japan Pension Service]. How it differs from the other exemptions is covered in National Pension exemptions and deferrals.

How it sits with sickness benefit, public assistance and unemployment benefit

The treatment splits by benefit. A Disability Basic Pension and a Disability Employees' Pension arising from the same cause are treated as a single pension[Japan Pension Service, receiving two pensions or choosing one].

CombinationOutcome
Disability Basic + Disability Employees' (same cause)Both are paid
Disability pension and old-age pension (before 65)Choose one of the two
Disability Basic + Old-age Employees' (from 65)Both are paid
Disability Employees' and sickness benefit (same illness)Benefit not paid (the difference if the pension is smaller)
Disability pension and public assistanceBoth possible, but the whole pension counts as assessed income
Disability pension and basic unemployment benefitNo statutory suspension

Sources: Japan Pension Service, "Receiving two pensions or choosing one", and Article 108(3) of the Health Insurance Act among others.

Sickness benefit needs care. The Health Insurance Act says it is not paid where a Disability Employees' Pension is available for the same illness[Health Insurance Act, Article 108]. If the pension is awarded retroactively, benefit you have already received is adjusted afterwards. The scheme is set out in sickness benefit continues after you leave the job.

Under public assistance there is no earned income deduction for a pension, so the amount is subtracted as it is. Pay does carry a deduction, so working still leaves you better off. The mechanics are covered in the income assessment when you work while on public assistance.

The benefit that is adjusted against the basic unemployment benefit is the Old-age Employees' Pension; there is no adjustment rule for disability pensions[Employees' Pension Insurance Act]. Note, though, that the basic benefit assumes you are able to work.

Allow roughly four to five months from claim to money in the bank

It takes about three months from filing the claim to receiving the pension certificate[Japan Pension Service, disability pension guide]. From the certificate to the first payment takes another one to two months.

  1. Fix the date of your first medical visit and get a certificate from that clinic or hospital
  2. Prepare the medical certificate for the disability recognition date (one year and six months after the first visit)
  3. File the pension claim with your municipality or a pension office
  4. The pension certificate in about three months, the first payment one to two months later (every even month thereafter)

Deadlines apply. Backdated payments are limited by prescription to five years, and a claim based on later worsening must be filed by the second day before your 65th birthday.

Three things you can do today. Check your pension certificate to see whether it is a before-20 case. If it is, review how your dependants are reported. And tell your attending doctor, before the next renewal, what adjustments your workplace makes.

The tax side of the pension itself (whether to file, how dependency is judged, the size of the disability deduction) is set out in disability pensions and tax.

Summary

  • The only pension that stops based on income is the basic disability pension for illness or injury before age 20. There is no income limit for the employees' disability pension and similar benefits.
  • For pre-age-20 illness or injury, the thresholds are previous-year income of ¥3,761,000 and ¥4,794,000. In terms of salary income, that's roughly from ¥5.25 million.
  • The disability grade does not drop merely because you are working. Make sure the renewal diagnosis reflects any workplace accommodations.
  • The pension itself is tax-exempt, but once employment income exceeds ¥620,000, the recipient is excluded from a family member's dependent status.
  • First check the pension certificate to see whether it falls under pre-age-20 illness or injury.
